Job Title: Case Manager -
Volunteer
Reports To: Director of Chaplains
The following statements are intended to
describe the general nature and level of work being performed. They are not
intended to be construed as an exhaustive list of all responsibilities,

SUMMARY:
The Union Gospel Mission Twin Cities is seeking to change lives with the
Gospel within the community. The Chaplain Department Volunteer Case Manager
provides support to the Director of Chaplains who oversees spiritual care
for all clients at the Mission. In addition, the Chaplain Department
Volunteer Case Manager develops friendships, gains trust, and guides at-risk
individuals in order to guide them through difficult situations.
DUTIES AND RESPONSIBILITIES:

As an integral member of the chaplain
staff, provides support for the Case Management program including the use of
client intake and tracking process software.

Develops partnerships with Community
Service Organizations of St. Paul and surrounding areas and liaise with
appropriate community boards, organizations, resources and the Union Gospel
Mission and connects these resources to clients.

When necessary, supports the evening
chapel program schedule including adherence to policies and procedures for
this area.

Provides spiritual care to Bethel Hotel
room residents including prayer.

Fosters a co-operative work environment
with other Mission programs to ensure that program goals and objectives are
achieved.

Responsible for maintaining client
confidentiality in accordance with HIPPA Privacy Regulations.

Compiles records and prepares monthly
reports.

Performs miscellaneous job-related duties
as assigned.
MINIMUM JOB REQUIREMENTS:
- Demonstrated Christian maturity and
passion for ministry.
- Active local church membership and
ministry involvement with demonstrated people skills (i.e., pastoral care
ministry).
- Be willing to participate in
on-going educational opportunities and certifications, i.e., CRP
certification.
- Must complete a background check
with no history of violence or sexual offenses.
- Must case manage at least one
client, meeting once per week at a minimum and totaling two hours
including documentation of meeting.
KNOWLEDGE, SKILLS, AND ABILITIES
REQUIRED:

Ability to analyze information and to
define and solve problems.

Ability to make administrative and
procedural decisions and judgments on case management issues.

Strong interpersonal and communication
skills and the ability to work effectively with a wide range of
constituencies in a diverse community.

Ability to implement program guidelines
and offer suggestions for improvement.

Competent in use of computers and
Microsoft Office software applications.

Ability to be self motivated and
demonstrate initiative.

Able to support UGM’s statement of
Christian faith and share that faith and knowledge with staff and clients.
WORKING CONDITIONS AND PHYSICAL
EFFORT:
-
Work is normally performed in a typical
interior/office work environment.
-
No or very limited exposure to physical
risk.
